Health Care
2 Self-insured plans with Preferred Provider benefits (PPO). A $300 or a $500 annual medical deductible and a $100 pharmacy deductible.
Dental Plan
$1250 annual maximum benefit, $50 annual deductible, one set of x-rays and two cleanings @ 100%, Orthodontic coverage for adults and dependent children with a $1500 maximum lifetime Orthodontic benefit.
Group Life
Benefit equivalent to Annual Compensation to a maximum of $50,000. Additional term life available at age-rated premiums up to $700,000 for employee; voluntary AD & D available for employee, spouse & children; dependent term life available for spouse and children.
Short-Term Disability
Covers a percentage of income after 16 business days of disability.
Long-Term Disability
Basic policy covers $1,000/mo. income. Additional coverage available to protect annual earnings up to maximum of $420,000. Benefit is 60% of insured monthly amount.
Flexible Benefits
Increase spendable income by deducting insurance premiums on a pre-tax basis; reimburse yourself with pre-tax dollars for certain health, dental, eye care expenses, and for child care costs.
Qualified Parking/Transportation Plan
Permits employees to pay for qualified parking or mass transportation costs on a pre-tax basis.
401(k) Plan
Deduct up to 25% of pre-tax income for retirement. 401(k) loans are available. You can choose to Self-Direct your 401(k) account. Over 50 catch-up contributions allowed.
Tuition
Reimbursement of 2/3 cost of tuition for approved courses up to $1,000 annually.
